This consent form confirms you understand what a shave-down involves when coat matting is severe. Please review each section and submit before your appointment.

Important Information

Severe matting can make humane brushing unsafe or painful, and a short clip may be the only safe option.
Shaving matted coats can reveal skin irritation, redness, sores, hot spots, or parasites that were hidden by the coat.
Existing matting may also cause skin sensitivity, clipper irritation, or uneven coat appearance while regrowth occurs.
Grooming around tightly matted areas carries increased risk of nicks, cuts, and minor abrasions despite careful handling.

By submitting this form, you acknowledge these risks and authorize Yelm Happy Tails Pet Salon to proceed with a matting-relief shave as needed for your pet's comfort and safety.
